Subject: Mandatory Pre-Reading Consent and Disclosure
Effective Date: March 14, 2026

1. Purpose: To ensure full transparency and compliance with the City of Las Vegas psychic arts regulations, Xango Wisdom requires every client to sign a Consent & Release Form prior to the commencement of any spiritual reading, divination, or entertainment performance.

2. Mandatory Consent Requirements. No reading shall be performed—whether in a commercial storefront, resort hotel, or private event—until the client has electronically or physically signed a document containing the following:

• Entertainment Disclosure: A clear statement that the reading is an exhibition of "Psychic Arts" intended solely for entertainment purposes under LVMC 6.12.090.
• No Professional Advice: A disclaimer stating that Xango Wisdom does not provide legal, medical, financial, or psychological advice.
• Age Verification: Confirmation that the client is 18 years of age or older, or has a legal guardian present to sign on their behalf.
• Fee Acknowledgment: A signed agreement of the total cost of the session to prevent "hidden fee" disputes, as required by city ordinance.

3. Record Keeping In alignment with privileged business licensing standards:

• Retention: Xango Wisdom will securely store all signed consent forms for a minimum of three (3) years.
• Privacy: Client data gathered on consent forms (Names, Dates) will be kept confidential and used only for legal compliance and internal record-keeping.

4. Refusal of Service Performers at Xango Wisdom reserve the right to refuse service if:

• A client refuses to sign the Mandatory Consent Form.
• A client appears to be under the influence of substances to the point where "informed consent" cannot be established.
• A client expresses a desire for the reading to replace professional medical or legal intervention.

5. Signage Cross-ReferenceThe Consent Form serves as a secondary reinforcement of the 8" x 10" signage required at the place of business by LVMC 6.12.070.
